Some common areas of concern encountered during this stage of life:

Laying the Groundwork

Legal issues, Medical/Confidential information – HIPAA, Medical Records organization, Insurance, Medication Management, Home Health, Nurse referrals, Choosing Physicians and Hospitals.

 

Making Choices

Review alternatives, Establish Goals, Coordinate Options and Choices with family members and support teams. Develop a personalized Plan of Care - a fluid working document.

 

Health Concerns

Identify health issues, Schedule, Attend, & Document Physician

Appointments, Coordinate Hospitalizations to Assure Safe Discharges.

follow-up: Rehab, Skilled Nursing, Home Health Post Hospital and Nursing at home.

 

Living Options

Home Care, Independent Living, Assisted Living, Memory Care, Respite.

Evaluate and recommend appropriate support and choices.

 

Communication

Monthly visits with client to reassess client status, family questions, referrals, & resources.  Frequency of visits varies based on changes in health status and requests by family.

 

End of Life

Education, Identifying appropriate Palliative &/or Hospice support to carry out client's wishes.
